UIL requires all students participating in athletics, cheer, and band to receive a pre-participation physical examination who are first year Junior High and first and third year High School (7th, 9th, & 11th) for the 2023-2024 school year. The following students will also be required to receive a physical examination:

1. Anyone who does not have a physical on file with NDISD Athletic Dept. for the 2023-2024 school year.

2. Any student athlete that is red flagged on the medical history (answers "Yes" to specific questions 1-6) and / or red flagged by the athletic trainer / school nurse.

THE PHYSICAL FORM MUST BE A CURRENT FORM WITH 2024 ON RIGHT HAND CORNER AND MUST HAVE ORIGINAL DOCTOR & STUDENT & PARENT / GUARDIAN SIGNATURE. PLEASE SIGN IN BLACK / BLUE INK ONLY (NO OTHER COLORED INK OR PENCIL). ALL UIL / NDISD ATHLETIC / MEDICAL FORMS MUST BE COMPLETED AND ON FILE AT SCHOOL PRIOR TO PARTICIPATION IN ANY PRACTICE, SCRIMMAGE, OR CONTEST, BEFORE, DURING, OR AFTER SCHOOL, INCLUDING AN ATHLETIC PERIOD.
